Gladwell has been working with the Ontario Disability Support Program (ODSP) to provide you with the supplies you require.

In order to qualify for the ODSP program, please complete the following steps:

1. Contact your physician or your local Ontario Disability Support Office and acquire a Mandatory Special Necessities Benefit Form (MSN).
2. Once approved with your caseworker, Gladwell will provide your caseworker with a quote for your monthly required supplies.
3. Reminder: have your caseworkers following information (Name, Phone/Fax number, and member ID number)
4. Once the quote is approved by the caseworker, they will fax us the information at (905) 660-9726
5. Once the quote is fulfilled and all paperwork is complete, a health care expert will contact you for further instructions.
